Herron-Riverside High School Athletics

The Marion County Public Health Department's ACTION Health Center will be hosting 4 sports physical days for $10 cash per student. If you have any students that need physicals, please feel free to bring them to the event (a flyer is enclosed with all the information.) The registration form needs to be completed, as well as the sports physical form or we will not be able to do it. Please have the parents sign the forms, as it is imperative for the students to receive the physical. You can make as many copies of the registration and physical forms as you wish.
